Apply for SIR Life Membership for the first time

Registration Process

Fill Registration Form

Complete the SIR Life Membership form with accurate personal, professional and residential details.

Read ByLaws & Accept Terms

Carefully read the ByLaws of the Society and agree to the Terms & Conditions before proceeding.

Upload Profile Photo

Upload a passport-size photograph with plain background. Selfies or outdoor photos are not allowed.

Upload Degree Certificate

Upload your Diploma or Degree Certificate in Radiology. Students may upload their Institution ID card.

Allowed: JPG, PNG, PDF — Max size: 3MB

Make Payment

Pay the membership fee via Credit/Debit Card, Net Banking or UPI.

Get Receipt

An automated online receipt will be generated and sent to your email once payment is successful.

Membership Approval

Your membership will be processed within 2–3 weeks subject to verification of your qualifications.

Receive ID Card & Certificate

After approval, your membership ID card and certificate will be sent via email in soft copy. No hard copy issued.
